In consideration for being permitted to utilize a facility or program of the Ozarks Regional YMCA, I, agree as follows:

• I AGREE TO INDEMNIFY AND HOLD HARMLESS the releasees and each of them from any loss, liability, damage, or cost they may incur arising from or related to my dependent’s presence in, upon, or about the YMCA premises or participating in any YMCA program whether caused by the negligence of the releasees or otherwise.

• I agree that if my dependent is hurt or my dependent’s property is damaged during my dependent’s participation in any activities, including, but not limited to the water trampoline, zip line, high ropes, and climbing wall, then I may be found by a court of law to have waived my dependent’s right to maintain a lawsuit against the parties being released on the basis of any claim for negligence.

• Also, I understand that this activity will not be made available to my dependent if I were to choose not to sign this release, and agree that the opportunity to participate in return for the execution of this release is reasonable.

I expressly accept and assume all of the risks inherent in this activity or that might have been caused by the negligence of the Releasees.  I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless Releasees from any and all claims, demands, or causes of action which are in any way connected with my dependent’s participation in this activity, or my dependent’s use of their equipment or facilities, arising from negligence.  This release does not apply to claims arising from intentional conduct.

I understand that should my dependent require transportation from camp due to illness, behavior problems or other reasons, I will be required to provide transportation in a timely manner as designated by camp administration.

In consideration of my dependent being permitted to participate in the activities; including, but not limited to the water trampoline, zip line, high ropes, and climbing wall, I further agree to indemnify and hold harmless Releases from any claims alleging negligence which are brought by or on behalf of minor or are in any way connected with such participation by minor.
